Assumption of Risk & Liability Waiver

1. Nature of Activity. Participation in baseball training, instruction, lessons, camps, clinics, games, and related activities at No Chance HQ ("Facility") involves inherent risks, including but not limited to: being struck by baseballs, bats, or equipment; collisions with other participants, walls, fences, or equipment; falls; sprains, strains, fractures, concussions, or other injuries; and in rare cases, serious bodily injury, permanent disability, or death.

2. Assumption of Risk. I voluntarily assume all risks associated with participation, both known and unknown, including risks arising from the negligence of No Chance HQ, its owners, employees, coaches, instructors, contractors, volunteers, and agents (collectively, the "Released Parties").

3. Release & Waiver of Claims. I hereby release, waive, discharge, and covenant not to sue the Released Parties from any and all liability, claims, demands, actions, and causes of action arising out of or related to any loss, damage, or injury sustained by the participant while on Facility premises or participating in any Facility activity.

4. Indemnification. I agree to indemnify and hold harmless the Released Parties from any loss, liability, damage, or cost they may incur arising out of participation.

5. Medical Authorization. I authorize No Chance HQ and its staff to administer first aid and to obtain emergency medical treatment for the participant if necessary. I am responsible for all medical costs.

6. Fitness to Participate. I confirm the participant is in good physical condition and has no medical condition that would prevent safe participation.

7. Governing Law. This agreement is governed by the laws of the State of Tennessee. If any provision is held invalid, the remainder of this agreement shall remain in full force.

8. Acknowledgment. I have read this agreement in full, understand it, and sign it voluntarily.

Photo & Video Media Release

I grant No Chance HQ and its affiliates the irrevocable, perpetual, royalty-free right to:

• Photograph and record video and audio of the participant during Facility activities;
• Use, reproduce, edit, publish, and distribute such media for marketing, social media, website, advertising, promotional, and commercial purposes;
• Use the participant's name, likeness, image, and voice in connection with such media.

No compensation will be owed for such use. I waive any right to inspect or approve the final use of the media.

Opt out of media use Check this box if you do NOT consent to photo or video use. The participant may still attend and use the Facility.
